Numbers Tell the Story: Pressure Ulcers and VAP

Objectives for program

Interpret NDNQI reports to identify a problem, implement corrective action and track progress over time.

Analyze, evaluate and revise interventions to accomplish desired outcomes using evidence based practice.

 

The purpose of this presentation is to show how NDNQI data combined with evidence based practice and appropriate interventions effectively improve outcomes.

 Abstract

 

This presentation will show how Riverview Medical Center, a community hospital in central NJ, used NDNQI data to improve outcomes related to a nosocomial pressure ulcers and Ventilator Associated Pneumonia (VAP).   In an atmosphere of limited resources and financial constraints hospitals are challenged to maximize each hospital day and use resources efficiently and effectively.  We will provide assistance in interpreting and analyzing data to identify areas in need of improvement implement recommended interventions based upon evidence based practice and justify possible expenses needed to achieve desired outcomes. 

 

Initial data from NDNQI showed a level of nosocomial pressure ulcers above the national mean.  An opportunity presented to participate in a statewide initiative addressing pressure ulcers using evidence based guidelines.  A pilot unit was designated, education was provided, unit resource staff were appointed and chart audits were done to monitor compliance with required and recommended approaches.  Follow up reports from NDNQI indicated significant decrease in nosocomial pressure ulcers on that unit.  The program was expanded to a second unit in May 2006 and plans are underway to further expand to a third. Staff nurses have been involved in conducting chart audits and participating in Just In Time education for their colleagues.  Progress in both units has been noted in the reports obtained from NDNQI and through internal chart audits.

 

With the current emphasis in nursing on Evidence Based Practice (EBP) nurses in all areas of practice have come to recognize and appreciate what this approach can do for patient outcomes. Using examples from projects undertaken at our hospital, information will be provided that addresses how EBP has evolved and effected outcomes positively.  An example of this occurred when critical care staff nurses were able to significantly decrease ventilator associated pneumonia over a period of 12 months.   In this example a multifaceted approach was utilized and included the phases of assessment, development, implementation, evaluation and follow up the project.  Riverview Medical Center was one of six hospitals in New Jersey recognized for this accomplishment.

 

NDNQI data can be an effective means to identify areas of improvement and track and trend progress to the desired goal.
