The Technological Evolution of Falls Incident Reporting

The objectives of this poster session are to demonstrate the effectiveness of online data collection and to demonstrate the evolution of the hardcopy data collection to a remote data entry system. The successful implementation of this process occurred because it was a required initiative from the Vice President   of Patient Care Services and Nursing Administration.  Online data collection improved reporting compliance within the institution and NDNQI and Joint Commission requirements were integrated into the data collection process.

The purpose is to show the evolution of the implementation of a remote data entry system to replace hardcopy data collection.  Barriers to the process included educating over 600 nurses and changing the mind set of the seasoned nurse. 

The implications for use included increased report compliance with real time data entry and the most current data collection tool always available to staff directly on the units.  The system provided real time notification of the incident for immediate follow-up by Nursing Directors, Nursing Quality Coordinator and Risk Management.   There is consistency of data collected throughout the Institution and the data meets Joint Commission and NJ State DOH regulations.
Reports generated by the system   can be individualized and reports can be developed ad hoc to meet specific needs of the Institution.  There is customization of the data collection forms depending on the needs of the individual units, ex. TCU Post fall assessment questions were added as required by the state DOH. The system can identify trends to initiate education or implementation of performance improvement indicators

The data is centralized in one data repository and is available to all users based on a specific security profile. There is no hardcopy.  Changes and updates can only be made by the Directors, Nursing Quality Coordinator and Risk Manager.
This presentation showed how the remote data entry system was implemented, the advantages and the barriers.
